Insurance code for dry eye scleral lens

Does anyone happen to have the medical codes for medically necessary scleral? I've finally got around to filling out a claim form and need to fill that in. Will probably have to call my docs, but figured I'd ask!

They wouldn't approve mine for dry eyes, but I have "astigmatisms with different medians" and that is how I got mine. We have VSP through fed BCBS. I paid $10.00 for mine.
