  • New here - Dry Eye Flare Up

    Hi All,

    I'm a 27 year old from Newcastle, Australia.

    I was diagnosed with dry eye about a year ago by my regular optometrist, he performed a fluorescent test and found that my eyes were drying in a matter of seconds. At the time he suggested I use Genteal Gel several times a day to alleviate the symptoms which until recently had worked brilliant (used approx 4 times a day, every 4-6 hours), I went back to him and he found my eyes to be much healthier about 2 weeks later. During this time I went about my day and didn't even think about my dry eye, it was effectively like being cured...

    In about September I had a moderate flare up, I returned to the optometrist as I was going on a cruise and felt I needed to have it looked into before I was on the wide open seas for 15 days with no treatment in site. On visiting the optometrist he did the fluorescent test and still found my eyes to be fairly healthy, again with good scores and no real dry spots, he suggested at the time that the flare up may actually be something else manifesting (ie. Cold or Flu), and strangely enough a few days later just before the cruise I did get a cold, and my dry eye symptoms went away. I went through one of the hottest summers in Aussie history and didn't have one flare up, wore contacts frequently and all in all still didn't feel dry eye was an issue.

    Fast forward to around 4 weeks ago...

    I had been doing my usual routine of sports most nights in the week, but hadn't worn contacts for quite a few days, and I woke up on the Saturday morning to put Genteal Gel in. When I put the gel in I noticed it stung a bit, but didn't think much of it, as I'd been out late the night before. I went away camping that weekend but followed my normal Genteal application and figured it would just go away.

    By the Wednesday of that week, my eyes were still fairly flared up, I returned to my optometrist again who stated that my eyes were indeed very inflamed and dry, and his suggestion was just to increase my Genteal usage as needed, he stated quite simply that I'd just have to live with it and make do with the gel so I could be comfy... I went to work the following day and found that my eyes were drying out in less than 2 hours, even with the Genteal, finding this to not be normal (especially considering the cooler Autumn climate), I decided to try and see another optometrist for a second opinion (this one had slit lamp cameras).

    On seeing this optometrist, she was shocked as to how inflamed and dry my eyes were, also at the fact that I was prescribed Genteal so heavily. She advised me that I had MGD, and that I should have been using a drop like Systane Balance all along to assist with the lipid layer as I was producing tears, just no oil. She immediately had me switch to systane balance 3 times a day and genteal only when I sleep and also prescribed FML (steroid drops) 3 times a day to reduce inflammation. A week passed and my eyes didn't feel any better, so I returned last week and was increased to 5 times a day (Occular Pressure was fine after the week). I also started taking liquid fish oil as well.

    The benefit in both of these appointments with this new optometrist is that with the slit lamp camera, she can show me the fluorescent images and video of my eye and point out the dark spots to me, it also allows us to track where this spots are. Now a week later, my eyes started feeling moderately better up until today, I'm due for another appointment tomorrow to see if there's been any progress, and the next step is a referral to a specialist if the steroid drops still aren't working...

    I have not worn contacts for a month, and have not had any lifestyle or dietary changes I could pin this flareup on, it seems like the Systane Balance barely works on me as well (I'm lucky to get 2 hours out of it, but at least at the moment can stretch it out with the FML) I do sometimes wonder if the FML could be drying out the systane balance as well. My eyes are fine in the morning, and I can usually make it to about 2-3pm before they get bothersome, but I guess I'm a bit worried all round as I usually wear contacts quite a bit through the week for sport (usually no more than 3 hours as a precaution from the dry eye), and dread to think that if this continues, I may be told not to wear contacts at all. Further to this, I've been lucky enough to have annual leave booked well in advance before this flareup, but return to work next week and while I feel better than my last day, I still haven't sat in front of a computer for 8 hours on leave.

    Had my follow up visit today, right eye has no dry spots on it (similiar to week 1) and the left eye has also made good progress, I'm now being tapered off the FML drops over the next 2 weeks, and then will have another follow up to see if I have any rebound flare up.

    Optometrist has also recommended me to try switching to Tears Again spray once off the FML to see if it's better for me (no preservatives).

    Fingers crossed things keep looking up...

      So a week later, and an interesting week it's been :/

      Yesterday was my first day back at work since I started with my new optometrist, and it was the worst day I've had since this first started flaring up.... Coincidence... Not likely!

      I work in a call centre and have for the last 5 years, always the same building, same air conditioning and nothing that I could think of that had changed. But yesterday I struggled to make it through the day with the dryness and pain I was in. The only change I can put it down to is moving to another area in the centre which I believe may have exasperated symptoms as it's more direct in line with the air conditioners, thus getting constant air... (I plan to talk to my manager about this and seeing if I can try and find a desk which is a bit more sheltered, however she's on leave)

      To top it off, after barely sleeping last night due to the dryness, I woke up this morning to blurry/hazy vision in my right eye which is usually my best eye (I could see out of it well enough to read, but it was a bit hazy), this cleared after about 30 minutes and everything seems back to normal now with no pain. I was hoping to get in with my optometrist just to make everything was still looking OK (was a little worried after this morning), however she's off sick for today with no replacement, and will be doing catchups for the next few days...

      So yeah, has been a fun few days, and now dread the thought that when I am back at work next week (full time), I'm going to be in constant pain and reaching for drops every hour...

        Iceman,
        I am very careful about what I eat. Usually. For Easter, I splurged and ate ice cream, cookies, ciabata bread with butter and chicken Marsala. The next day my eyes were a MESS! (Plugged oil glands.) The biggest mess they have been in months. Everyone has different triggers, but I am guessing I cannot have ice cream or sugar, and probably no butter or easily metabolized carbs. Just a thought. Good luck!

          Hi iceman_oz,

          Have to tried moisture retaining glasses? We can get them here in Sydney, I rang the company originally and they gave us the name of a local Optician that sold them they were very helpful. http://www.wileyx.com.au/ 1300 782 990.

          Good luck, hope you get to see the Optometrist soon and that she refers you on if it's not resolving.

            So I had an unrelated flare up last night where I started seeing flashes in the corner of my eye, went to the emergency room and then an eye specialist today, in the end they put it down to an optical migraine (all my tests came up fine).

            I utilised the opportunity to talk to the specialist about my dry eye, and she believes I have blepharitis, which is the first time anyone has actually discussed this as being a cause (and matches up with a lot of my symptoms).

            Also discussed eyelid massages, cleanliness and warm compresses which she suggested for all (good to know she had a good grasp on treatment).

            When we got to drops though, I couldn't get a clear direction... At first she suggested I go back on Genteal stating that if it worked for so long, I should probably see if it works again when starting compresses etc. On one hand I think this could be good, as when I used it for the first 18 months, I had no real symptoms of dry eye, nothing like the last 2 months where I come to know what all of you know as dry eye.... On the other hand, I worry about using a preservative gel long term, and also the fact that it stopped working and I was having to use it more often when this all started (about every hour).

            She also questioned the use of FML for the dry eye (although my eyes don't seem anywhere near as inflammed as what they were back then)

            She has given me a trial of Poly Gel (preservative free) to use at night, but didn't really give me a clear direction on what to try during the day non-preservative wise... The optometrist recommended Theratears Gel last time, but this was difficult to find so I tried the drops instead which were less than effective (plus being single use and not resealable - thus very costly).

            I have another appointment with the optometrist, so will run a lot of this new found info by her and see what she recommends next... And also booked an Ophthalmologist appointment booked in June (the earliest I can get in), so will keep this post as an ongoing diary of what works and what doesn't.

              Theratear gel are eye drops but thicker . usually very easy to find in chemist . u can get an authority script from gp or opthomologist . It's will cost u 36 for any number of drop boxes. so if u use one box per day you will get 30 boxes for $36. Standard for any brand.

                Thanks soaps, that's useful to know... Will discuss that with my optometrist tomorrow night as she is a dispensing optician.

                I take it you are Australian as well, and also noticed you were posting in the NAC thread, make sure to keep us posted on how that goes as it certainly looks interesting...
