Healthy meibomian glands but no discharge

Hi,
last week I had lipiscan and meibomian graphy. My tear osmolarity was 302-304 (300 is the beginning for dry eye). My doctor said I have great meibomian glands anatomy. They are not clogged, damaged or there is no change in oil structure. By the way, I have dry eyes since 2020. However, they just don’t discharge properly. When I look at the mirror, I make massage and see the oily layer but during the day discharge is very rare and almost no discharge. I don’t understand what causes this. Does anyone else have this problem?

I have a similar thing (But overall worse - more dropeout, some clogging).
Its kind of frustrating, it feels like «its getting worse», while I dont know how I can treat it effectively.
