I think it is really a continium of disease but some good doctors seem to have pointed agaisnt this. This is from a spectators view so to say, my own opinion is it is a continium in either fashion for either type of patient. Once it starts to get controlled though maybe it halts and even reverses this process? That's what my non-scientific view would infer.
