  • Method for treating MGD

    I recently came across a patent on patentsonline.com called method for treating meibomian gland dysfunction. There are lots of different elements to it, but the part that interested me most was the bit about microdermabrasion or exfoliation to remove dead cells from the meibomian gland openings (extract below).

    Has anyone heard about this before? It seems like a good idea to me.

    [0058] A second embodiment of the invention (FIG. 5) employs microdermabraision or exfoliation to remove any cells or cellular material that may have overgrown the gland opening. Microdermabraision is a process that was developed for use in dermatology to remove dead skin cells. As shown in FIG. 5 a probe or tip 330 is equipped with an abrasive surface 310 that is adapted to scrape the skin. The abrasive employed is usually a diamond power or other suitable material, well known to those skilled in the art. An inner tube 320 having a central bore 325 includes holes defining openings 330 through which a fluid such as air is pumped. An outer covering 335 surrounds the tube 320, but at its lower edge extends slightly lower and is spaced from the abrasive surface 310 and a space is defined between the lower ends of the respective tubes 320, 335. The outer covering is connected to aspiration, vacuum, and/or suction that operates as described herein below.

    [0059] In operation, the clinician would place the abrasive tip 310 in contact over the gland orifice creating a seal between the tip and the skin. Movement of the probe 330 would cause the abrasive 310 on the bottom of the tip to separate the cells from the skin and the aspiration, suction or vacuum would extract the cellular material from the vicinity of the gland opening. In addition, depending upon the obstruction, aspiration, suction and/or vacuum alone may be sufficient to extract the obstruction.

    [0060] Additional features may also be providing to the microdermabraision tip such as a heating element 340 which could be placed in the outer covering 335 near the tip. In addition, the inner tube 320 could be equipped such that ultrasonic energy could be delivered to the obstruction as discussed herein above.

    [0061] Another embodiment of the invention may employ a chemical agent to clean the gland margin and to remove or exfoliate cells from the meibomian gland orifice. For example Ophthaine.RTM. or a similar pharmacological agent may be employed to assist in removing epithelial cells from over the gland orifice. A probe similar to that shown in FIG. 5 may be employed, except that the inner tube will deliver the chemical agent and the suction applied by the outer covering will be used to evacuate the used chemical agent and cellular material mixture away from the gland margin. Similarly, the heating and vibrational features discussed above may also be included.

    Where could you get this treatment?
    I come from the UK too, and cant imagine our "bargain basement national health service" paying for that !!!
    Sounds interesting - I have often wondered if it was possible to use some "micro- surgical" procedure to unblock the glands If you find anyone prepared to do this , I for one would be interested

      I don't know if it's possible to have this procedure done and I've got no idea where to find out! I'm sure it wouldn't be available in the UK on the NHS anyway! Has anyone in the States ever heard of this?

      Have you seen any good doctors in the UK about your MGD?

        In short- no, not yet
        I think the secret is to make enquiries as to ophthalmologists who have a special interest in ocular surface disorder
        I found a comprehensive list of members of the "Medical and Scientific Advisary Board" for the Tear film and ocular surface societywww.tearfilm.org/content/msaboard.html
        That might be a start
        It lists names of people from many countries including some from Britain- I presume they are ophthalmologists
        So far that's the best i have come up with
        If u come across anyone good u could let me know
        Ian P has the name of a man in Edinburgh called David Crystal I am not sure if he is an eye doctor or an Optometist or what - but several people have found him very helpful and anyone Ian would recommend would be good Im sure

          Hi Stella

          I've thought about seeing David Crystal in the past but he's only an optometrist and I've seen other optometrists, and they're pretty limited in what they can do.

          I'm going to see John Dart at Moorfields in a few weeks so I'll let you know how I get on with him. If he can't help, I'm considering taking a trip to New York.

          I would love to find out more about microdermabrasion/exfoliation though as it seems to make a lot of sense to me.

            I expect that you would have to go to the states to get this or maybe someone private in the UK- still might be pushing it.

            Its prob the same with all the new treatments that will be available in the future. Im fully prepared to go to the states to try them as i know theres no hope of getting it in the UK. And nothings gonna chnge untill they get rid of the NHS. Which isnt likely to happen anytime soon becuase ignorant people think it is good coz its supposed to be 'free'

              retinoic acid intended as dermabrasion via chemical peel

              I like what I'm reading here about mechanical dermabasion to remove the metaplasia that can overgrow meibomian openings, but I believe the concept itself is not new, in that Dr. Tseng has been attempting to re-open sealed meibomians via chemical peel, using retinoic acid ointment, for a long time. I may have reported that I'm aware of a few patients for whom this worked well, even though I was on the ointment for years, and experienced no change. . .Anyway, the concept seems smart. . .Let us hope the patent is gotten and the device gets introduced into clinical practice. . .Then we'll have to double-up on the gland stimulation strategies. . .
