List of Questions for Doctor

Hi! I have seen on here before a good list of questions to ask your doctor and now I can't find it! I'm going to the doctor tonight...could anyone tell me how to find that list- it would be really helpful for me for my visit tonight. Thank you!

1) How is my tear quantity?
2) Do you see any dry spots?
3) Can you check for staining?
4) Can you press on my meibomian (oil) glands? Are they releasing oil? Is the oil clear and flowing freely? If not, what does it look like?
5) Do you see any signs of allergies?
6) Do I have any lid closure problems (e.g., lagophthalmous)?

What is going to be used to establish baseline health of my corneal epithelium and used to monitor if the therapy is working?

And just as important is a list of meds be taken, the environment at home and work, air conditioning or heat make issues worse, ceiling fans at night or overhead at work, etc.
