intracanalicular plugs lost?

Has anyone ever had one of the intracanalicular plugs lost inside? Also, has anyone had their doctor insert another plug over the intracanalicular plug (instead of irrigating it out) when the intra plug no longers occudes the tears fully?

There have been reports of the Herrick style Lacrimedics plug migrating.

Yes there have been reports of doctors inserting plugs without confirming the location of previous plugs. Theoretically, every reasonable effort should be made to locate the previously inserted plug.
