perscription drug coverage for Restasis

Is Restasis covered under most insurance plans? I'm about to order my first batch (have been using samples from the eye dr).

It is for me

I've got United Healthcare and prescriptions are ordered thru Medco.

If the prescription is written for use twice a day, you can actually use one vial for both appications making them last longer.

Restasis was covered under my drug plan when I took it. Initially it was a Tier 2 with a relatively low co-pay $20 or so. Then my employer changed the plan and made it a Tier 3 with a $50 co-pay. So unfair because there is no substitute! Much depends on the plan administrator as to what is covered and at what dollar amount.
