Thank you, Dr. G, for prompting qs. on scleral lenses. This was the catalyst I've needed.
I have always wondered whether scleral lenses, if tolerated well, can eliminate the "menthol sensation" that some DES patients experience. That sensation is the only disabling symptom I have, but it is completely disabling, in the sense that I am unable to function either indoors or outdoors, for more than very brief periods, without benefit of moisture chamber glasses or goggles. My TBUT has been between 0 and 1 sec. for about 9 years, now, but I do produce tears at a nearly normal volume. (My Dx is endstage MGD, i.e., little-to-no detectable meibum secreted, with lots of glands covered by metaplasia.)
I once tried large/scleral bandage lenses for a spell, when my opthalmologist felt these might reduce the awful menthol sensation (which is like a strong wind blowing in the eyes, even when the atmospheric air is rather still), but these soft bandage lenses did not help at all, and, in fact, worsened the problem.
How are sclerals, on this front?
