Dry Eye
Knowledge of 'dry eye' problems in general is central to a decision on the likely safety and efficacy of therapy with any TCL hydrogel, silicone or scleral. TCLs are generally only considered in the treatment of dry eye patients when all other modes of therapy have failed. Thick low water content or silicone rubber lenses are preferable for dry eye patients with associated ocular surface disease. For most patients, lenses alone do not alleviate dry eye symptoms and frequent instillation of ocular lubricants is still required. Sealed rigid gas permeable scleral lenses are the best option in these cases.

Hydrogel lenses
Hydrogel lenses can quickly make a painful eye comfortable, they are easy to fit, readily available in many types and parameters including tints and are cost effective. There are several disadvantages since they do not perform well on dry eyes nor do they enhance the vision with irregular corneas. They can rapidly accumulate deposits, the tear film in the eye needing such a lens is likely to be abnormal. Frequent replacement is required which increases costs.
