    Hello. I am someone who has been undergoing a combination of IPL in hopes of improving my Meibomian gland dysfunction. I would like to share my experiences, as IPL can be expensive.

    - I am a 26 year old male with white skin. I am saying this as it sound like IPL has some risks for darker skin types.

    - I have 28% gland loss in my left eye and 58% in the right eye. The right eye is the problem - the left eye is not so noticeable.

    - My diagnosis came from the only specialist in Oslo, the Dry Eye Clinic. However, they admitted they could not guess the original cause. We discussed incomplete blink reflex or nocturnal lagopthalmos, but they could not say for sure.

    I originally started with only warm compresses, combined with eyelid massage and eye drops. However, it was not enough. So I found an optician that offered IPL. The price is 1500 NOK each time - about 152 us dollars. I don't know if they have problems with taking foreign clients, but I could call them and ask if anyone is interested.

    As of today I have done 3 IPL - This is with E-eye, using the Toyos protocol where you wear googles and they only engage the light around your eyes, not directly on your eyelids. The entire process of each IPL worked like this:

    1: Use an electronic device called Blephex on your eyelashes. It is like a massager that removes blepharitis from your eyelashes. This was the most uncomfortable part of the procedure. It felt like my eyelashes was being tickled. Not painful, but uncomfortable.

    2: Apply a gel around your eyes, wear the googles, and actually perform the IPL. Each hit of the IPL was slightly painful, but only for a split second. Like jerking back in surprise from the warmth on your skin.

    3: Wear a heated therapearl eye mask for 10 minutes. They warmed it in the microwave.

    4: Meibomian gland expression. Given numbing eye drops, then followed by the glands manually being expressed. Not painful and less uncomfortable than the blephex.

    It is now about 10 days after my third IPL, and my eyes have gotten better. My right eye was such an issue that I stopped reading books completely, but now I am slowly starting to enjoy literature again.

    I would say I am no longer depressed from the dysfunction alone and despairing, so I will heavily recommend it. Now, I do want to make it clear my right eye is not perfect, and it does feel different and not "normal". But the IPLs have made it so that the warm compress and massages are way more functional, and I feel better for a while after.

    My routine goes like this, recommended by the Dry Eye Clinic:

    - Wash eyelids. I use blephasol. It's a solution you drop on cotton wipes (included) and drag it across.

    - Wear mask for 10 minutes. This has actually given me small bumps on the eyelid, which I believe are styes, so I am going to start using cotton pads warmed with hot water on my eyelids to protect from direct contact with the mask. Hopefully it will work. EDIT: Using cotton pads and warming them with warm water before putting them on my eyelids works - the mask does not seem to give me styes anymore, and the expression afterwards still works.

    - Eyelid massage using fingers. I use my fingers, based on this video: BEST EYELID MASSAGE GUIDE: How To Do Meibomian Gland Expression For Dry Eyes, MGD, And Styes - YouTube

    After this, combined with the 3 IPLs I have, I feel better.

    I will try it a fourth time and report back.

    UPDATE: I will be doing my 5th or 6th IPL in august or September. I will update a week after doing it, to let it take some effect. Sorry for late with updating.

    Because IPLs are expensive, I do recommend trying to follow the mask and massage routine first. That way you won't pay right away for IPL when it could work alone. But for me the IPL explicitly made it more functional in expressing glands.

    You know you are doing the massage correctly when you see this white "slime" appear in your tear duct.

    The Dry Eye Clinic also recommended me dry as drops as necessary - Artelac preservative free one-use containers, but I think the expressioning of the gland is significantly more useful.

    UPDATE 2 OCTOBER: As said above, my right eye is the problem, and it gets a physical feeling that is probably from the Meibomian Gland Dysfunction.

    HOWEVER, now that I have done IPLs, warm compress and meibomian gland massage works pretty well. I clean my eyelids with either anti-blepharitis foam in the shower or do blephasol wipes on my eyelashes, then the heated mask for 10 mintues, then massages. After this my eyes feel good for many hours and

    I recommend trying to do at least 1 ipl, then start doing the heated mask routine and massaging eyelids. Note that the massaging has to be done correctly, more of a squeeze and hold with your upper and lower eyelid to do it correctly. Then gradually do more IPL until it works.

    My right eye still makes me sad sometimes, but it is a lot better. So I just wake up 15 minutes earlier a day to do the routine

    33 male from Oslo (Norway) here.

    Its so great to hear IPL treatments have worked for you, and that your condition is improving, gives me some hope.
    Its also kind of refreshing to read about someone going through a similar treatment plan as me (all other experiences I read about uses eyedrops and treatments with different names).

    I think my condition is similar to yours, I also have MGD. Ive had 3 IPL sessions, but no real sign of improvement yet for me.
    During my last IPL session, the doctor told me 3 glands was expressing oil in my right eye, and 4 glands in my left eye.
    Even though it hasnt given me any significant effect yet, Im probably aiming for at least 6 IPL treatments (I dont know what else to do, so I might as well..).
    For reference, I have struggled with Dry Eyes since May 2016, but it became a lot more severe a year ago, now using screens is uncomfortable at best (And I work in IT).

    I dont know about you, but Artelac generally doesnt give me much relief. I find using HYLO Gel/Comod works a lot better (If you want to try something else).

    I can definately relate to the depression you mentioned. Or maybe its rather: Being frustrated, and fearing not being able to get better.
    In any case, I hope your next IPL session does wonders for you!

        I’ve had 5 sessions of IPL in the U.K. plus Meibomium gland expression. It has definitely helped me. I still use lots of drops and do warm compresses but the burning feeling is so much better.

        I hope you have further improvements soon.

          I wouldn't do it if you have freckles as your skin will be more sun sensitive
          Perhaps get some high SPF and a rich moisturiser
          It just lightly burns the skin. That's all. You can dress it up but it just causes mild burning and therefore exfoliation. But you can exfoliate in other ways.

            Hey everyone. I will be updating the thread next month or september. I have been sick and have had other priorities, but by August or september I will have had done 5 or six Intense Pulse Lights.

                  For how much minutes have you doing ur massage with ur fingers? I have try seeing the video that you posted, but I'm not sure if I'm doing it right... any advice? I mean, I cant see the lipids.

                  Also, do you think is a good idea do blink exercises after the massage?

                    The way I was told to do the massage is to push the eyelids up or down and hold it for ten seconds. I start at one corner of the eyelid, then press it up or down with a finger, and hold it for 10 seconds. Then I do the same for middle and right inner of eye, on both upper and lower eyelid. Most youtube videos only drag for 1-2 seconds, which I do not recommend. I hold for 10 seconds, this is AFTER blephasol and 10 minutes of heated mask. You know you are doing it right when your eyes get blurry.

                    I wish I could find a way to record myself how it is done, I could try to do it with my phone, need someone else to record me.                     It is apparently important to look in the oppsite direction with your eye, to not hurt your cornea. So when press lower eyelid up your eye look ups to the ceiling, look to the floor when pressing upper eyelid down. The goal is to not press the eyelids into your eye, but to press them down and up, not fingers pressing inwards towards eye.

                    Sorry for the late response. Any questions and I will answer.

                    To summarize:
                    - I apply anti blepharitis cleaning. You can get it as shampoo or wipes. I have a shampoo called demodex or wipes called blephasol
                    - Wear a heated microwave mask for 10 minutes
                    - Do massage with fingers. Press my finger to my eyelid and hold up or down for 10 seconds. Do it on outer corner of eye, middle then inner. So 3 per eyelid, 6 total on each eye for 12 total. My eye becomes slightly blurry, which means the glands are being expressed.

                      Hi - Just to say I did 3 sessions of IPL including the expression with the medical tweezers, after the first session my eyes felt better, but when I finished all of them, I swear it's like I never did anything at all... This treatment was a waste of money for me. I read a book by an American doctor called: The Dry Eye Remedy by Dr Latkany, it's the most amazing book ever, he's got decades of experience with dry eye, when no one would be talking about it, then I had an online consultation with him.. he told me these expensive treatments dont work and can be potentially harmful for the eyes.                         Could you tell me more about it? I mean:
                        1. What's ur dry eyes cause?
                        2. How was ur Schrimer and TBUT before and after the IPL?
                        3. What kind of IPL u did? (The bottom and top eyelids or just bottom - with protection inside the eyelids or off)
                        4. How was ur xpression's glands?
                        5. How much time you did the last session?

                          1- I still dont have an answer to this, I have MGD for sure, but it all started with office light sensitivity 10 years ago my eyes would burn terribly while in the office.. but just recently I found out that this is migrane! People with migrane can suffer with light, sound, cold.. I had no clue I had migrane, but yes, after seeing 3 different neurologists this i what I have, my brain does not like office lighting, computer screens. I suspect after years of exposure at work, eyes burning, my glands were terribly affected by this. I dont think a doctor will tell us the cause, we are the ones who know when this all started possibly?
                          2- My Schrimer tests are always good, my problem is not in producing tear, but clogged glands. I spoke to an Ayurveda eye doctor in India, he asked if I had oily skin and oily hair, I said yes, then he said: so you dont have a lack of oil production, you have blockage problem, you need unblock your glands.
                          3- I did the bottom only, wearing protection glasses, nothing inside the lids.
                          4- After the IPL the doctor squeezed my glands, but I only felt better after the first session. IPL only gives a temporary relief, it's not worth the money. We are so desperate for a solution that we pay whatever just to feel normal again! I would not spend a penny on IPL, a total rip-off.
                          5- The session lasts 5 mins!

                          In my opinion, the secret is to do the expression correctly at home. I use an electric eye mask, while I'm wearing it I press gently against my eye lid doing a massage, like Lipiflow does. Keep pressing it gently, this lasts 20 mins. Then I do more eye massage in front of the mirror with the cotton bud ( wet with saline water) and I squeeze the glands with my eye tweezer ( same as doctors use, I actually bought one and use it very carefully. I can feel the oil coming back : )
                          I also posted something here on the main page, on the Introduction page, explaining what I do when my eyes are terribly dry! Let me know if you read it. Aline

                              Thank you for ur advices. I already try the old IPL Toyos protocol (the bottom only), but I'm reading about a lot of ppl who didnt have good results with this but feel some difference with the Periman protocol (the top and bottom eyelids and with protection inside), so I'm thinking about to give it a chace.

                              Anyway, i saved the book and gonna try to give it a chance.
