  • MGD and dry eye

Hi I'm new so sorry if this has been asked before. Is it important to know what ingredient is missing from my tears so I can buy the correct drops ? I have MGD but had them expressed and doc says they are flowing nicely but I still have severe dryness and pain. I believe the eyes make 3 different lubricants naturally so does treatment depend on which one is lacking?thanks

Have your doctor checked your eyes for goblet cell deficiency?

Lipids flowing out of meibomian glands with manual pressure doesn't indicate they are expressing out with the normal pressure of a blink. You could be a partial blinker. Also the composition of meibum might not be as it should be. Have you been checked for hormonal imbalances (sex steroids)?

There's another condition that causes dry eye like symptoms but is often overlooked. It's conjunctivochalasis. You should visit an eye doctor who specializes in dry eye treatment.

Also there could neuropathic sensations of dryness for some reason. It's very hard to determine what's actually going wrong unless you are thoroughly checked by a dry eye specialist.

      Thanks for your reply Milo007
im learning a lot on here about what to ask my doctor next time I see him. There are so many tests never offered to me but if you don't ask, you don't get I guess.

Yes exactly as the old saying goes.... a baby who doesn't cry gets no attention even from his/her mother. So basically you want to investigate as much possibilities as you know about before you start losing your glands.

Eye drops are only good for easing the symptoms for 4-10 hours. I dont think it will solve the problem.
You should investigate the cause then will be time to think of a solution.
