      Do you know if its a run-of-the-mill staph infection or if it's methicillin-resistant staph aureus (otherwise know as MRSA, pronounced Mer-suh)?

      Community acquired MRSA infections are on the rise and are resistant to several antibiotics including methicillin, penicillin and others.

      A quick google search for "ocular MRSA moxifloxacin" revealed this as the first hit: "An alarming 76 per cent and 82 per cent of ocular MRSA isolates were resistant to moxifloxacin and gatifloxacin, respectively." If you indeed have MRSA, this could explain why the moxifloxacin was ineffective. I know for systemic MRSA, the gold standard for treatment is intravenous vancomycin. I don't know what is routinely used for ocular MRSA.

      You definitely need to find out if you have MRSA or just plain old normal staph.

        i had the culture report too.
        It says the bacteria is resistant to following anti bacterials
        amoxycillin
        cephalexin
        cefixime
        nalodixic acid
        oflaxin
        ciprofloxacin
        pefloxacin
        lemofloxacin
        prufifloxacin

        It says the bacteria is sebnsitive to moxifloxacin and methecilin.
        But the thing is i used to have oflaxin regularly for sometime.Still infection didnt go away.Now report says its resistant to oflaxin.means bateria has become resistant to it bcoz i have used it.I doubt if i try other antibacterials, the bacteria might get resitant to them also.Lets see...im to consult my Optho on monday.
        Meanwhile i have started AZYTHROMYCIN orals so that infection dont spread much in other parts of bodyl

          amoxycillin = penicillin-like
          cephalexin = cephalosporin (closely related to penicillin)
          cefixime = cephalosporin (closely related to penicillin)
          nalidixic acid = synthetic quinolone
          oflaxin = fluoroquinolone
          ciprofloxacin = fluoroquinolone
          pefloxacin = fluoroquinolone
          lemofloxacin = fluoroquinolone
          prulifloxacin = fluoroquinolone

          Ok....do you see a trend here? Basically the strain of staph you have is resistant to many penicillin/penicillin-like and fluoroquinolone antibiotics. The two you listed that the report said the bacteria are sensitive to are methicillin (penicillin-like) and moxifloxacin (fluoroquinolone). So basically, if the bacteria is resistant to several drugs in a class, I would be hesitant to take ANOTHER drug from within that same class. In my opinion, your concern that you may develop additional resistance to one of these drugs is well founded. I was going to suggest a macrolide, but it seems your doctor has already prescribed one since azithromycin is a macrolide. So that should be reassuring. Take ALL of them and follow up carefully. Ask your ophthalmologist a lot of questions. If he can't resolve the eye infection ask for a referral to an infectious disease specialist.

            hi.
            thax for the info abt antibiotics.

            i have been feeling nasal discharge and congested nose from 2 decades...since sjs..just 2 years back i had cauttery done which opened within 2-3 months.There after i stated to have this infection...Again i had cautery 1.5 mnth back..the infection went away for 15-20 days...no nasal discharge too...then again i felt cauttery is opened up..and nasal discharge and infection started...I went to have cauteruchecked.Doc said he feels that cauttery isnt opened up now...But i think its opened.I will be taking another opinion soon.

            I went to optho again and he said, the infection seems to be within lacrimal gland..If its within lacrimal gland and cauttery is closed then how can nose have constant congestion ?

              The nasal congestion is probably due to inflammation. Your immune system has been recruited to the site of infection, but the inflammation and swelling may not be purely in the lacrimal gland. I'm sure the sinuses around the eyes and subsequently the nose and throat are inflamed as well. You may want to see an ear nose and throat doctor.
