An eye doctor was so kind to recommend me this comprehensive report,
''Emerging strategies for the diagnosis and treatment of meibomian gland dysfunction:

compiled by 11 eye doctors including:
French: Prof. Christophe Baudouin German: Prof. Gerd Geerling and Prof. E. Messmer
Perhaps it could help you know your options.
Have you heard of non-invasive TBU using (advaced/latest technology) Keratogaphy (item 5.3)?
Confocal microsopy: I plan to do it as I have never done it.
It can detect more info on MGD and demodex issues, according to the report.
I have had no luck in finding a solution-driven doctor so I need to keep learning and exploring experiments.
I asked one doctor if BlephEx, probing, IPL would help me. He said yes.
But the one who performs two of them says NO need.
