    I have no corneal staining or dry spots and tear break up time of over 10 seconds. But I get a lot of pain from my eyes I think due to low tear production. 2mm and 3 mm on the schirmers test. That's without anesthetic. Any one else had this problem due to lack of clinical signs?

    Yes, I have this problem. Signs and symptoms do not match.

      My eyes were damaged from UV exposure and that started off my dry eyes and pain. I just went to my Opthalmologist after being on Restasis for 5 weeks. I went from corneal staining and a TBUT of 1 second to no corneal staining and a TBUT of 8-10 seconds.

      Problem is that my eyes still hurt and I got the same dismissive response as well. "The signs are looking good, but that doesn't mean there is no pain, the symptoms are wildly out of proportion with this disease" is what he said, when I asked what I could do about the pain he gave no options and talked about a study done ten years ago about patients seeing their primary care physicians for the the of pain, which he couldn't remember the name of "Psycho something" he said. Probably Psychosomatic I'm assuming, meaning that the pain is real, but caused by emotional feelings.

      With the growing interest and study of corneal neuralgia in the field of Opthalmology, it made me pretty mad that my eyes could go through so much trauma and he could just toss me aside as the pain coming from my head.

      It's pretty frustrating, but I'm in the same boat, maybe try a new doctor? I saw an Optometrist who was much more compassionate than my Opthalmologist and I might be able to convince her to let me try some different therapies, that could help with the pain.

        What I have read is, such pain is real but most eye doctors can NOT solve it as they are NOT trained.
        So maybe good to try other doctors?? In London, you have dry eye center, maybe they have better knowledge??
        Or try to get tests by confocal microscopy, most Uni. hopistals have it, to check if cornea nerve damage?
