I did it but didn't care for the required contact info part at the end. I'll gladly skip a $10 gift cert to maintain my privacy, particularly when it comes to this sort of thing.

I agree with you PotatoCakes. It involved giving someone my name and address, and so I passed. Would be glad to fill out the survey without the personal information attached.

Oh dear - I'm really sorry. I should have thought of that. I made it through to the end but didn't fill that out - but must have been in a hurry as I didn't realize the survey doesn't get submitted without it. I see there is no explanatory language about what happens to the contact information. I had told them that I accept survey links about dry eye so long as there is no marketing attached... but giving personal information certainly has that implication so I will contact them and find out.

OK here's the scoop -

1) The survey now requires only state of residence and email address
2) The source of the survey assured me they do not keep the contact information for marketing purposes, it is simply so that they have the option of verifying the validity of submissions (seems reasonable to me that they would want a way to spot-check at least to ensure they are legitimate).

So if you don't mind going through it again, sure appreciate your efforts Heaven knows we don't have enough companies interested in finding out what we want/need, so it's great when we can help them along.

I completed the survey but was a little annoyed with the section where you are supposed to put the cost of the treatment before insurance. On some of these, I really didn't want to guess. There was no option for "don't know" or "n/a." I think a drop down menu with price ranges and a "don't know" option would be better.

Also, my survey wouldn't allow me to proceed until all of the price fields were filled out, whether I had used the treatment or not, so I had to put in zeros.

Rebecca, I'm not sure if you have contact with the survey requester, but these small changes might make it a better survey.
