  • IPL with Dr. Assil

    Hi.

    I thought I should update everyone now that it has been 24 hours since my first IPL treatment with Dr. Assil. Here is my experience.

    I arrived at 1:30 (my appt was at 1:45). The office is nice the people working there are very friendly. I was not taken back until 2:45. Once I got taken back I was met there by Angel, the "practice development director." She talked to me for a while. Eventually Dr. Assil came in and took me to an examination room ( I was in the IPL room, where the machine is). He did various tests including a TBUT, I think I was a 5 & 6 on those from what I saw in his notes, I should have asked. He examined me for a while and asked all sorts of questions about prior treatments, I think to make sure that everything else had been tried. Next he expressed the glands and then had an assistant take various pictures of my eye to.

    He then went to do a LASIK procedure and came back an hour and 20 minutes later to see how long the relief from the expression had lasted. It had lasted about 45 min to an hour.

    He then examined the pictures to make sure my eyes could handle the treatment since I had had LASIK 2 years ago he wanted to make sure that I had healed correctly. Finally he did the IPL treatment, did two sweeps of it and then took me next door to the exam room to do the expressions again. He said the oil coming out from the left eye was "murky" not "toothpastey" but then he said the right was more toothpastey (which he said is not ideal).

    Dr. Assil did tell me to stop my doses of Restasis (which I have been on for about a year).

    I still don't have another appointment. scheduled. Angel said she would call me to check up on me and schedule another appointment then.

    As for the results. My eyes were definitely whiter than they normally are right after the treatment. ( I have very red eyes all day they are usually worst in the morning and at night). This morning I woke up and for the first time in a while my eyelids hadn't stuck to my eye overnight. I also noticed that my eyes were less red than normal and I actually had sleep in the inner corners, again this has not happened in about two years.

    The redness did come back a little bit as the day has gone on. I do feel better than normal especially on the left eye which is my worst one. The right is pretty much the same... still dry, but not as painful as usual.

    Please let me know if you have any questions about my experience and I will try to update this when anything changes.

    If anyone else sees Dr. Assil for IPL please post! I am trying to decide who to continue with for either another treatment or future maintenance (depending on how I feel after this last treatment). I thought Dr. Toyos was great but is now only in Memphis.....Dr. Assil is only a 40 min drive for me....

      Pati, a few things about your post.

      1. Very informative post and especially so because you are a post-Lasik. That will be interesting to watch. I believe different causes may react differently to the treatment. I have Sjogrens along with Lasik and I think I would not get any use of the treatment. I don't believe I have any oil to express because of the Sjogrens.

      2. It's exciting that you are feeling some better already.

      LOL. He probably did more than one Lasik procedure. It's amazing how they can fit us in.

      4. Autumnn---Can it hurt for you to see this doc and decide if you like him and is it worth it etc.?

        Hi Lucy- How are you?
        Great advice- I think I will do exactly that- meet with him, etc and then decide. Dr. Toyos says that he "certified him" and is confident in him....but, I still want to know how many procedures he has done, etc. Luckily I have around a month to three months to decide. I was expressed by Assil's staff last time and the woman (a female eye doctor) who did that was great- I even could go out on a limb and say that her expression of the glands seemed better or equal to Dr. Toyos. So, if Assil can work the machine then many of us are in luck...

          Autumnn, I think you found a jewel in this attendant that can "express" the meibomian glands! I hope it all works out for you. Lucy

            Sorry it took a while for me to reply, I have been traveling.Thanks for the response and for sharing my excitement. I know our conditions aren't all the same, but I feel that it can give us some hope.
            I am very curious to see how my eyes continue. They are starting to get back to how they felt before the treatment.

            Autumnn -- I will keep you posted on how my next visit is. I should have another one in the next couple of weeks.

              Thanks Pati- Remember this treatment takes a lot of time- many need 4+ treatments before they see results...good luck and keep us posted.

                Well- I schd a 5th apt (my left eye is still an issue, it has improved for sure but not enough) and it is with Dr. Assil! It was so hard for me to decide between flying to Memphis for a doc I really trust and going to Dr. Assil who is less than an hour's drive away. As I mentioned, Dr. Toyos assured me that he has properly trained and certified Dr. Assil and is confident in him....I am schd for 2 weeks from now. I will keep you all posted and will detail a comparison between the two doctors for the forum

                  Autumn,

                  i am excited for you!

                  It is always difficult to swtich Dr.'s, even many of us post lasik people are reluctant to leave the our Lasik "Dr." but if things work out you will wonder why you didn't do it before.

                    Thanks Bernadette. I am excited to see this new Dr. too. I feel so close to being better- I can almost taste it. I feel like Dr. Toyos' treatment dragged me up the difficult hill and now I need one more good push to get going down the other side. Lets hope that is the case.... Like I've said a million times- I want so badly for this to be the answer for as many of us as possible.

                        another question re IPL

                        Hi,
                        I started having problems in March of this year and diganosis was blepharitis. It's been a battle ever since.

                        The problem I have is I can't seem to get a definitive diagnosis to know if I may even be a candidate for this. I have been to 3 opthamologists and one corneal specialist. Only one tried to express my glands and told me very little oil came out. I don't even know what THAT is supposed to mean. By my own research I have determined I have MGD and low tear production. I am post lasix (10 years) and post accutane (16 years). I had to request getting tested for Sjogrens because apparently nobody else has thought about that. Sometimes I feel like I am more knowledgeable than the Dr's I have been seeing. The Sjogrens results are pending. The best person I have seen is an optometrist. He seemed the most knowledgeable but $100 a visit and not covered by my insurance.

                        It is so frustrating. I am planning a trip to NY to see Dr. Latkany in Nov. (just so I can finally get a diagnosis by someone who has a clue) but that seems so far away. Sometimes I have a few good weeks where I feel almost normal and then a major setback where I feel I can hardly function.


                        Would Dr. Assil be able to actually diagnose me? It looks like he is in CA which is alot closer than NY!

                          Hi Sabrina -

                          Welcome. You sound like many of us when we turn up here. Be assured that you are in good company and you are going to learn a lot and feel a lot better as a result. I can personally attest to that.

                          It sounds like you have done a good job of research and your self-diagnosis sounds pretty plausible (of course I am not a Dr but do have a lot of personal experience here) Your background sounds a lot like our dear Autumnn. I would PM her - she is awesome. As far as Arizona, there is AmyK that is from Az and may have some ideas for you as far as Drs.
