IPL with Dr Toyos

    How do the docs check the meibomian glands? did you have other docs tell you the glands were blocked before seeing toyos? all the docs keep telling me that everything looks to be in not all that bad shape but my eyes are driving me crazy! I had lasik though so I feel as though they really don't have a clue whats going on, it was a gamble and I lost big time... now I have to gamble with all these post lasik dry eye treatments and hope that I eventually get some luck.

    • the ipl pretty much covers the entire face, and the gland expression is in the lower gladn......
      the first time he did the gland expression was pretty intense, i think they were pretty plugged.
      one other dr. said they were plugged, quite frankly i dont think most docs want/like to express them, they would rather prescribe something and get you out, been my experience.

      i notice much more comfort after second treatment.
      i was also give asazite to help clean them up.
      he looked thru a microscope to see the glands , i assume......so far i'm pleased, but not getting hopes up, ive tried so many things.....

        thanks for the info, I think the lady that did only two shots under the eye on me has got it wrong and I should be getting at least half the face done... but still not really sure what to do, don't think I'll go back to this particular shop though.

        • aaron, when u say lady??....whatta u mean??.....opthalmologist, right??

          the procedure with toyos includes several zaps on both side of face and around the bottom eyelid, it isnt pleasant to say the least, and also toyos does lid expression with a qtip that is also pretty intense.....my glands were completely blocked, for whatever reason, and i'm sure it takes some intense work to get them going again.....this is a medical procedure....i'm not trying to be your big brother , but i wouldnt trust it to anyone but an m.d.
          my eyes feel comfy today, the best few weeks ive had in years, still dont know what causes mgd, i'll ask next time.....
          i know it was so intense i had to stop him to catch my breath, but i'm glad i did it....

            had it done at a cosmetics place, no opthamologists are doing this anywhere close to where I live. I think I may have to fly in to see Assil in LA. Waiting to hear back from his office.

            • just got off the phone with dr. assil's office, the IPL is costing $750 dollars per treatment with 6-8 treatments required.

              • When I spoke with Dr. Toyos' office last week, they quoted me $500 per treatment... if airfare is the same cost to Memphis and L.A. then the savings would add up with Dr. Toyos.

                  airfare is way cheaper to LA for me, it would cost me a lot more to go to Memphis. I'll have to wait for an aircanada seat sail.

                  making a vacation to LA out of it would be nice but I'd feel really bad like I'm getting ripped off again if I went in and saw him. well I'm figuring that there's a good chance I'd feel that way at least.

                    • Well, I booked myself an appt. for ipl through my dermatologist... her nurse performs the procedure... they booked me for the full face tho... $250 per treatment which takes 1/2 an hour.

                      My first appt. is Jan22... so here we go!! Hopefully I get at least some benefit...

                        It was $375 per treatment at the assil eye institute over the summer when i did it?

                        So i guess that rules me going back for more

                        I was going to have IPL done where i live, and since my glands aren't blocked really (caps), i don't think the gland expression makes much difference for me anyway.

                        I feel that it might have helped my right eye, left still really bad though.

                          I hope you get some benefit because that would prove you don't need Dr. Toyos to perform this, he claims there's a special 'technique'..not so sure about that.

                          • Yeah, it would be AWESOME if I get some benefit... especially since it's so much cheaper than flying to Memphis ( or L.A.)!

                            At the very least I hope it benefits my face some ( my face looks perfectly normal, thanks to Metrogel, but my rosacea makes it burn and itch like mad around my nose argh!)

                            • I think it might depend on how blocked the glands are, he expresses them VERY hard after IPL, which will make the caps pop and loosen, over time with ipl they become less solidified i guess.

                              I have heard before that IPL has helped those with rosacea that have ocular rosacea, from other websites, and DR toyos noticed that Ocular rosacea befitted from IPL when they were using it ONLY for the face... that is why he started using it in the first place. However, i think this may have halped those who have milder symptoms and glands arent as blocked.. you may need the expression as well if glands are really blocked, although IPL may still help.. over time.

                              I have inflamed (conjunctivitis), of inner eyelids not eyes, Dr toyos said it was a different mechanism, so it might not be improved by ipl to the face.. it wasnt. But its still ocular rosacea as far as im concerned no other explanation.. allergies dont make sense.

                              • The thing that made me finally decide to skip Dr. Toyos for now is that I now suspect that my primary problem is aqueous deficiency. I now am quadra-plugged, and there is no hint of overflow... eyes are still quite dry, but better than I was when only double plugged.

                                Also, both my optometrist and corneal specialist said my MG's are only mildly abnormal, and therefore they didn't think THAT is likely the main cause of my dry eye. My optometrist told me last Sept that my glands show "mild capping"... Now in Jan my corneal specialist said they look pretty good, and that there IS oil coming out...

                                However, since steaming my lids and expressing my glands DID give me some major improvement in the burning I had, I am hoping that I might squeak out a tiny bit more improvement with IPL.... who knows though... but at least with the derm it'll only cost me $1500 max. ... and at this point, it's money I'm willing to lose, even if it ends up not helping at all. But maybe I'll be lucky...
