Punctual plugs: how to find the best fit?

Plugs for severe DES
I've plugged my lowers today. The caps are not bothering me much, maybe because I've used contacts for many years at a time when they were not as advanced as they are today. Hope the plugs will help me with my severe DES. I will also start with Restasis. Will be posting how things go from here.

MGD: Should I get my uppers plugged?
I already have my lowers plugged and they helped tremendously. I've been told I have MGD and evaporative dry eyes. How helpful would it be to get my uppers plugged too? My reasoning is it'd take more time for tears to evaporate.
Anyone with MGD who experienced relief after double plugs?...

Schrimer scores worse after treatments...?
After 10 months of restasis, 6 months of doxy, 8 months of warm compresses + expression 3x a day for 15-20 min, 8 months of being quadra-plugged, and 3 months of serum my eyes still feel like crap.
What's most strange is after all this treatments my schrimer scores are worse than before...

4 plugs so far no tears
So I went to have a temporary plugs put in today. I had all 4 ducts plugged so I thought I was going to be overflowing. So far I had not been. In fact I placed eye drops in and noticed it went away after 1 min. Is this normal? How long did it take for any of you to experience help after plugs?

I have MGD and Aqueous Deficiency. I'm considering punctal plugs but I've heard arguments saying that punctal plugs make MGD worse and some saying that it would help me. What do you all think? Any professionals care to give their 2 cents.
